A case of implant rehabilitation using bone grafts

Mrs. Manascu first came to our clinic complaining of the absence of her back teeth in the inferior arch and mobility of the bridge on her upper arch teeth. Because of these problems, she could not chew and, aesthetically speaking, despite her very joyful character, she felt very insecure and did not smile anymore.

The situation seemed very complex, especially on the upper arch, where the few residual teeth needed to be extracted (either because they were decayed under the gums and therefore irrecoverable or because they had roots with non-removable bolts and apical endodontic lesions of large dimensions that are not treatable).

In addition, the maxillary sinus was very pneumatized, resulting in a lack of bone in the posterior areas and with a very hypertrophic Schneiderian membrane (very undesirable situation in the procedure of maxillary sinus lift for bone regeneration).
The procedure involved first the clearance of the upper arch (extraction of all the teeth and roots) with simultaneous grafting of vertical and horizontal bone and uplift of both maxillary sinuses and, after a 5-month healing period, the placement of 6 implants; on these plants, on the same day, a fixed provisional denture was placed that was kept on for a couple of months to evaluate the tightness of implants. At this point, a fixed ceramic-metal denture was designed and built.

For the lower arch, which showed relatively lower difficulties, four implants were placed at the same time, which, after 1 month of healing, were used to make 2 bridges for 3 teeth on each side. Finally, the lower frontal group, once the teeth had been devitalized correctly, was rehabilitated with a fixed bridge.

This treatment has allowed the patient to regain both her complete masticatory capacity, self-confidence and proverbial desire to smile.
